The project seeks to improve asthma management for children and reduce the number of hospitalizations and emergency department visits due to asthma though a multi-trigger, multicomponent intervention. Self-management education, medication management, environmental assessment, and asthma trigger remediation will be conducted by GHHI staff, for a targeted set of Priority Partners Managed Care Organization (PPMCO) members who have had a hospitalization or emergency department visit due to asthma in the previous twelve month period. If outcomes are met, PPMCO will use cost savings to issue success payments.
