The project seeks to improve asthma management for children and adults and reduce the number of hospitalizations and emergency department visits due to asthma though a multi-trigger, multicomponent intervention. Clinic staff and community health workers at Baystate Pulmonary Rehabilitation, provide clinical care and home visiting. Project managers and data staff at Partners for a Healthier Community coordinate clinical and community based services as well as providing data management, analysis, and support. Home assessors and technicians at Revitalize CDC and the City of Springfield provide environmental assessments and remediate asthma triggers. If outcomes are met, Health New England will use cost savings to issue success payments to upfront investors.
