The Green & Healthy Homes Initiative (GHHI) will advance four Pay for Success (PFS) projects benefiting low-income families through providing comprehensive services and addressing social determinants of health. These awards, through the Social Innovation Fund, will support the Community Foundation for Greater Buffalo (Buffalo, NY), Health Net of West Michigan (Grand Rapids, MI), Le Bonheur Community Health and Well-Being (Memphis, TN), and Partners for a Healthier Community (Springfield, MA). Each of the PFS projects focuses on helping asthmatics in low income communities through comprehensive services that include home-based care and addressing environmental triggers of asthma such as pests and mold.
